HealthLift Pharmacy Services (“HealthLift”, “we”) operates health-lift.com. You can reach us at helpdesk@health-lift.com or PO Box 520190, Salt Lake City, UT 84152.
What this notice covers
This notice covers information you give us through this website: our forms, links we send you, and the pages you visit. Protected health information we handle for our clients is also governed by our HIPAA Notice and by our agreements with those clients.
What we collect
- What you type into a form: your name, email address, phone number and organization, and the details the form asks for.
- Claimant identifiers on the claim forms: a claimant’s name, date of birth, date of injury and claim number.
- Technical information: a one-way hash of your IP address and browser type, used to prevent abuse. We do not store your IP address itself.
- Page and click events: which page you were on when you clicked a phone number or submitted a form, and any campaign tags in the link you followed. These events contain no personal information.
We do not use advertising cookies or third-party trackers.
How we use it
To respond to your request, to verify, redirect or bill the claim you asked about, to send you a receipt, to prevent abuse of our forms, and to understand which pages help people reach us.
How we protect it
Claimant identifiers are encrypted with a key held in a separate key vault, are never written to logs or analytics, are never placed in a web address, and are never shown back to you after you submit. Access is limited to staff who need it to act on your request.
Who we share it with
With the payor, pharmacy or PBM involved in the claim you asked about, when that is needed to act on your request; with service providers who host our systems under contract; and when the law requires it. We do not sell personal information.
How long we keep it

Your choices
You can ask us what information we hold about you, or ask us to correct or delete it, by emailing helpdesk@health-lift.com. Some information must be kept to meet legal and contractual obligations.
