Using this website
health-lift.com is operated by HealthLift Pharmacy Services. By using it you agree to these terms. If you do not agree, please do not use the site.
Information on the site
The site describes HealthLift’s services in general terms. It is not an offer, and nothing on it commits HealthLift to a price, service level or payment term. Those are set only in a signed agreement. Figures marked “illustrative” are simulated and do not describe any client’s results.
Forms and agreements
Some forms include an agreement, for example the terms that apply when a claim is redirected to a payor’s PBM. When you tick an agreement box and submit, you accept the wording shown at that moment. We keep a copy of that exact wording with your submission.
You must be authorized to share any information you submit about another person.
Links we send you
A link we send you may open a form with some details already filled in. Treat it like any other message about a claim: do not forward it. Links expire, and each can be used a limited number of times.
Acceptable use
Do not try to disrupt the site, get around its security, submit information you are not entitled to share, or submit automated or bulk requests.
